Required Paperwork For All Athletes

Student-Athlete safety is the number one priority of Civic Memorial High School and the CM Athletic Department. In order to ensure that all of our student-athletes are medically cleared and safe  to participate in interscholastic sports, the following paperwork must be on file at Civic Memorial High School for each student-athlete.  All student athletes must turn in a hard copy of the athletic physical signed by the appropriate physician.  All other paperwork must be completed through the online platform (click on the tab/link below to submit necessary paperwork).  Parents/students need to read through the the information above prior to both parties signing off on the paperwork.

  • Current physical, signed by a doctor, that states the student-athlete is medically cleared to participate in sports (students entering grades 10 through 12 need to turn in the IHSA athletic physical.  Students entering 9th grade need to turn in the athletic physical as well as the incoming 9th grade physical).
  • Proof of insurance. In lieu of a student-athlete being covered by a parent/guardian's insurance, a parent/guardian can purchase insurance for their son/daughter at 1st Agency by clicking on this link.  A student covered under their parent/guardian insurance must submit a Waiver of Insurance to participate in CMHS Athletics.
  • A signed Civic Memorial Student-Athlete Code of Conduct. (Will be posted online and made available in late June 2017.
  • A signed Illinois High School Association (IHSA) Concussion, Performance Enhancing Substances, and Asthma Form (one form, often referred to as the "IHSA Form" or "IHSA Concussion Form".)

Athletic Fees

*Special Note:  The 2020-2021 Athletic fees will remain the same as they were for the 2019-2020 school year.  Please refer to the 2018-2019 fee schedule below to obtain information regarding the fee necessary to play a sport at CM or TMS.

Athletic fees at Civic Memorial High School and Trimpe Middle School were changed by the Board of Education in the spring of 2017. Below are athletic fees for student-athletes and CM and TMS. As you can see, there is a graduated increase over the next three school years. 

On the below chart, "Reduced" refers to if the athlete and his/her family receive reduced price lunches as part of the Federal School Lunch Program, and "Free" refers to if the athlete and his/her family receives free lunches, as determined by the school district's food service coordinator. 

We do not want athletic fees to ever be a reason that an athlete chooses not to play a sport. If the fee is an issue for any athlete, please come and see Mrs. Govero or Mr. Hannaford in the Civic Memorial High School main office to discuss the fee and options that may be available. 

Turn-In Process

You may download and read any of the necessary athletic forms here on the website and then submit that you have read the information by "signing off" using the link posted above. No student-athletes will be allowed to try-out for a team, practice for a team, or play for a team unless all of the required documents are on file in the athletic office.
